What you will do:

Conduct strengths-based assessments of adults with care and support needs in accordance with the Social Services and Well-being (Wales) Act 2014.

Develop, implement, and review person-centred care and support plans tailored to individual needs and outcomes.

Manage a varied and complex caseload, ensuring timely interventions and effective risk management.

Undertake Mental Capacity Assessments and contribute to Best Interest decision-making processes where appropriate.

Respond to safeguarding concerns and undertake safeguarding duties in line with statutory guidance and council procedures.

Work collaboratively with health professionals, care providers, housing services, and partner agencies to coordinate support.
