What you will do:

  • Conduct strengths-based assessments of adults with care and support needs in accordance with the Social Services and Well-being (Wales) Act 2014.

  • Develop, implement, and review person-centred care and support plans tailored to individual needs and outcomes.

  • Manage a varied and complex caseload, ensuring timely interventions and effective risk management.

  • Undertake Mental Capacity Assessments and contribute to Best Interest decision-making processes where appropriate.

  • Respond to safeguarding concerns and undertake safeguarding duties in line with statutory guidance and council procedures.

  • Work collaboratively with health professionals, care providers, housing services, and partner agencies to coordinate support.

  • Arrange, commission, and monitor care packages, ensuring services remain appropriate, effective, and provide value

Salary range
