What you will do:

  • Undertake Care Act 2014 assessments, reviews, and support planning to meet individual needs and promote independence.

  • Complete safeguarding enquiries and manage complex adult social care casework.

  • Apply strengths-based approaches to empower adults and support their wellbeing and independence.

  • Develop person-centred care and support plans in partnership with adults, families, carers, and relevant professionals.

  • Work collaboratively with health colleagues and partner agencies within an integrated service model.

  • Undertake Mental Capacity Assessments and apply the principles of the Mental Capacity Act 2005 where appropriate.

  • Manage a diverse caseload effectively while ensuring compliance with statutory duties and timescales.
