What you will do:
- Conduct comprehensive assessments of the needs of older adults in the community and develop appropriate care plans.
- Collaborate effectively with colleagues, family members, and other stakeholders to ensure the delivery of high-quality social services.
- Provide advocacy and advice for older individuals and their families to navigate social services framework.
- Monitor and review cases regularly, adjusting care plans as needed.
- Keep accurate records and prepare reports regarding individual cases, adhering to relevant legislation and guidelines.

Requirements for the Social Worker:
- You must have a degree or equivalent in social work and hold a current registration with Social Work England.
- Significant frontline experience working within Social Services in the UK as a qualified social worker is essential.
