What you will do

  • Assessment: Complete Care Act assessments, reviews, and person-centred support plans.
  • Case Management: Manage a varied caseload of older adults with functional and organic mental health conditions.
  • Dementia Support: Work with individuals living with dementia and early-onset dementia to promote independence and wellbeing.
  • Mental Capacity: Undertake Mental Capacity Act assessments and Best Interest decision-making.
  • Safeguarding: Carry out safeguarding enquiries and contribute to adult protection procedures.
  • Legislation: Apply the Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983, and associated legislation in practice.
  • Multi-Agency Working: Collaborate with Community Mental Health Teams, NHS professionals, GPs, carers, and voluntary organisations.
  • Care Coordination: Support effective care planning and risk management through multidisciplinary interventions.
  • Record Keeping: Maintain accurate case records and prepare high-quality assessments and reports.
