What you will do:

  • Facilitate safe, effective, and timely hospital discharges, ensuring smooth transitions from hospital to community care.

  • Undertake Care Act 2014 assessments to identify long-term care and support needs.

  • Complete strengths-based assessments and develop person-centred support plans.

  • Work collaboratively with NHS colleagues, community health services, and partner agencies.

  • Conduct risk assessments and implement proportionate plans that promote independence and safety.

  • Undertake Mental Capacity Assessments and apply the principles of the Mental Capacity Act 2005 where appropriate.

  • Participate in safeguarding enquiries and ensure adults at risk are protected through appropriate interventions.

Salary range
