Care Coordinator

Salary: From £27,000 + bonus & shared on-call (DOE)

Hours: Full-time, Monday - Friday plus on call

Apollo Home Healthcare is seeking an experienced Care Coordinator to support the smooth delivery of person-centred homecare across the Northwest. You will manage rota planning, care package coordination, and act as a key contact for clients, families, and professionals.

This role is based in our Heywood office with regular regional travel. A Full UK Driving Licence and your own vehicle are essential.

Duties and responsibilities:
  • Rota management and scheduling for care teams
  • First point of contact for clients, families, and healthcare professionals
  • Monitoring care delivery to ensure compliance with care plans
  • Attending multi-agency meetings with ICBs
  • Supporting the shared on-call rota (no hands-on care)
What you need:
  • Full UK Driving Licence + own vehicle
  • Experience as a Care Coordinator
  • Strong scheduling and software skills (e.g., People Planner)
  • Knowledge of person-centred care
  • Excellent communication, organisation, and problem-solving abilities
  • Ability to build strong relationships with staff and external partners
