  • Develop, implement, and review personalised recovery plans focused on individual goals and outcomes.

  • Support service users to access treatment, health services, education, training, employment, and community resources.

  • Conduct assessments and monitor progress throughout the recovery journey.

  • Facilitate group sessions and recovery-focused interventions that encourage peer support and shared experiences.

  • Work collaboratively with healthcare professionals, partner agencies, and support services to deliver coordinated care.

  • Support harm reduction initiatives and promote safer lifestyle choices.

  • Identify safeguarding concerns and follow appropriate procedures to protect vulnerable individuals.
