This ongoing role pays £33 hourly and requires a full-time commitment of 37 hours per week. The Prestatyn Community Resource Team (CRT) is where you'll make a significant difference by working with a blend of health, social care, and third sector teams, all striving to deliver integrated community services.

What you will do:

* Conduct appropriate and proportionate assessments with individuals and their families to help achieve realistic and desirable outcomes.

* Collaborate effectively within a multidisciplinary team comprising Social Workers, Occupational Therapists, and other social care staff.

* Actively engage in partnership working to ensure seamless service delivery within the community.

* Manage and prioritise your own workload efficiently while demonstrating self-motivation and initiative.

* Maintain professional registration with the Care Council for Wales and commit to continuing professional development.
