We are seeking a dedicated and experienced Care Coordinator to join our growing team. This role is essential to ensuring that our clients receive the highest quality of care and that our support workers are well-supported and motivated. This is an exciting opportunity for a proactive individual with experience in domiciliary care who thrives in a dynamic environment and has a passion for leading others.
As the Care Coordinator, you will act as the primary point of contact for clients and support workers, providing critical operational support and overseeing daily care logistics.
Key Responsibilities
Client Care- Develop and manage care schedules to meet clients' needs, ensuring coverage for staff absences.
- Monitor and address cancelled care hours with Team Leaders.
- Oversee high-quality, person-centred care and maintain strong communication with clients and families.
- Host team meetings to promote collaboration and problem-solving.
- Recruit and match support workers to clients in partnership with recruitment.
- Collaborate with HR and Clinical Leads to address staff issues and ensure ongoing training.
- Coordinate with internal and external stakeholders to ensure seamless care.
- Supervise care and medication standards and maintain accurate records.
- Report on client progress and share service impact with stakeholders.
- Plan and deploy resources efficiently across care packages.
- Support scheduling, billing, payroll, and participate in the on-call rotation.
- Strong customer service skills with experience in domiciliary care or a similar environment.
- Outstanding communication and logistical planning skills.
- Ability to work both independently and as part of a team, with a calm approach under pressure.
- Mentoring and coaching experience to support the development of other team members.
- Own transport is essential.
- Basic Salary up to £27,500 depending on experience
