What you will do:

- Conduct comprehensive Care Act assessments to cater specifically for extra care sheltered accommodation voids.

- Perform carer's assessments and reviews, ensuring all care needs are being met effectively.

- Complete scheduled reviews and serve as a point of contact for service users.

- Undertake safeguarding enquiries diligently to ensure the safety and well-being of all clients.

- Present cases at various panels, including those involving joint funding, and prepare statements, working alongside the legal team for court hearings.

- Participate in duty systems, representing the service at GP Multidisciplinary Team meetings.

- Liaise with the Care Support team at Leacroft Lodge and other Extra Care schemes, ensuring seamless delivery of services.
