What you will do:

- Conduct comprehensive annual reviews for individuals as part of the Promoting and Independence Review Team.

- Take on case-related responsibilities such as completing Mental Capacity Assessments.

- Ensure all reviews follow a strength-based and holistic approach, promoting independence every step of the way.

- Collaborate effectively with other team members to develop and deliver tailored support plans.

- Keep meticulous records, reflecting on the outcomes and effectiveness of available support services.

Requirements of the Social Worker:

* Degree or equivalent in social work

* Current Social Work England registration

* Significant frontline experience in Social Services within the UK working as a qualified Social Worker
