What you will do:

* Conduct Holistic Assessments and Develop Support Plans : Perform comprehensive assessments to establish eligibility and create personalised support plans that uphold wellbeing and independence.

* Ensure a Least Restrictive Approach : Complete positive risk assessments designed to prevent hospital admissions and support individuals in their own homes.

* Apply Legislation and Practice : Use your knowledge of the Care Act, Mental Health Act, Human Rights Act, and Mental Capacity Act to guide practice decisions and ensure compliance with legislation.

* Safeguarding : Act with authority to protect adults at risk of abuse or neglect, ensuring their safety and protection.

* Teamwork and Duty Rota : Engage in the duty rota and collaborate with various agencies to deliver integrated care.

* Ensure Accurate Record Keeping: Maintain detailed records through council IT systems, adhering to administrative and financial procedures.

* Share Expertise and Mentor: Provide mentorship to colleagues and students, sharing expertise to enhance team capabilities as you progress.

* Embrace Inclusive Practice: Incorporate consideration for diverse needs arising from age, gender, sexuality, health, race, religion, culture, and life experiences into your work.

Requirements:

* Proven experience working with adults with complex mental health needs.

* Registration with Social Work England (SWE) is essential.

* Proficiency in social work methods to promote change, independence, and wellbeing.

* Strong IT skills for effective case management.

* Knowledgeable in relevant legislation with the ability to apply it to practice.
