What you will do:
* Hold and effectively manage a complex and varied caseload while following policy, procedures, guidance, and legislation.
* Prepare written reports, statements, and representations for judicial proceedings, including applications to the Court of Protection.
* Use professional judgement and evidence-based tools for complex decision-making and risk management.
* Communicate effectively in high-pressure or complex situations, representing the department at multi-disciplinary forums as needed.
* Directly work with service users, carers, and families according to interagency safeguarding policies and procedures.
* Assess and navigate care for individuals with eligible social care needs in collaboration with health and other agencies.
* Plan and coordinate responses to meet individual needs, working closely with carers, advocates, and representatives.
* Monitor and review support plans to ensure service standards and financial resource management.
* Jointly work with Social Care Assessors and participate in practice education for student placements.
* Undertake AMHP duties if qualified, including participation in the Out of Hours AMHP rota.
* Lead safeguarding inquiries and work cooperatively across multi-agency boundaries to improve practice standards.
