Qualified Social Worker - Adults Integrated Discharge Hospital Team
Location: Rotherham, UK
Salary: £33.00 per hour (Ongoing Contract)
Working Pattern: Full-Time or Part-Time
An exciting opportunity is available for a Qualified Social Worker to join the Adults Integrated Discharge Hospital Team in Rotherham . This ongoing role offers a competitive hourly rate of £33.00 and the flexibility to work full-time or part-time . Reporting to the Team Manager , you will play a vital role in supporting vulnerable adults through the hospital discharge process, ensuring high-quality care and a collaborative, multi-agency approach.

Key Responsibilities:
* Deliver personalised social care services for vulnerable adults, supporting statutory social work functions.
* Conduct assessments and investigations, producing evidence-based reports to inform care planning.
* Develop and implement support plans that meet individual needs, collaborating with other agencies as required.
* Monitor and review care plans to ensure high standards of service delivery and adjust them when necessary.
* Empower clients by promoting self-advocacy and ensuring their voice remains central to service provision.
* Provide information, advice, and support to clients, carers, and families throughout the discharge process.
* Work in partnership with multi-agency teams to ensure a seamless and coordinated discharge.
* Undertake safeguarding investigations and ensure appropriate interventions when required.
* Contribute to the development of preventative strategies to reduce hospital readmissions and promote independence.
Requirements:
* Qualifications: Degree or equivalent qualification in Social Work .
* Registration: Current registration with Social Work England (SWE) .
* Experience: Demonstrable experience working within hospital discharge or adult social care settings.
* Skills: Strong assessment, communication, and organisational abilities with a commitment to person-centred practice.
