What you will do:

* Work collaboratively with the Multi-Disciplinary Team to plan, monitor, and evaluate care strategies for adults and carers.

* Support daily operations, including case triaging, attending huddles, and participating in safeguarding processes like MASH, MARAC, and MAPPA.

* Conduct Best Interest assessments concerning the Deprivation of Liberties and carry out needs-led assessments, care planning, and multi-disciplinary evaluations.

* Provide guidance and help develop social resilience in adults and their families, focusing on hospital admission prevention.

* Communicate complex assessments and care plans effectively to diverse stakeholders using appropriate methods, mindful of information governance requirements.

* Submit funding applications, working with health colleagues to advocate for resources and ensure compliance with advocacy needs and rights.

* Engage with community forums to promote older adult, physical, and mental health social care, enhancing your professional expertise.

* Enquire into safeguarding concerns with a personalised response, aiming to improve individual outcomes.

* Actively participate and lead where necessary in multi-disciplinary meetings, ensuring clear presentation and organisation of documentation.

* Demonstrate dedication to professional development and the cultivation of supportive, resilient practice environments for peers and new colleagues.

As an ideal candidate, you possess strong knowledge of UK social work regulations, proven experience with vulnerable adults, and exemplary communication skills. You are registered with Social Work England, with relevant qualifications and a commitment to ongoing professional development.
