What you will do:

* Undertake comprehensive assessments and care planning in compliance with the Care Act 2014.

* Work collaboratively with service users, families, carers, and multi-disciplinary teams to deliver exceptional care.

* Support individuals to access appropriate care and services that encourage independence and personal growth.

* Manage a varied and engaging caseload while adhering to safeguarding policies and procedures without missing a beat.

* Conduct Mental Capacity Act assessments, ensuring that decisions are always in the best interest of the service user.

* Maintain accurate and detailed case records and reports that reflect your attention to detail.

* Advocate fiercely for service users, ensuring their voices are heard and respected in all decision-making processes.

Requirements:

* You must be a Qualified Social Worker (QSW) with active Social Work England registration.

* Demonstrable experience working with adults with physical disabilities.

* Comprehensive knowledge of relevant legislation, including the Care Act 2014 and Mental Capacity Act.

* Strong skills in assessment, risk management, and problem-solving.

* Ability to work independently and thrive within a collaborative multi-disciplinary team.

* Exceptional communication, organisational, and report-writing skills.
