Social Worker - Adults Review Team - Huddersfield - £35 Hourly - Ongoing

This is an exciting opportunity to join the Adult Review Team in Huddersfield as a Social Worker. As an experienced and qualified Social Worker, you will be part of a dynamic team working across both North and South Kirklees. The role focuses on conducting comprehensive Care Act assessments and reviews for adults aged 18 and over within care home settings. What you will do:

* Conduct Care Act assessments and reviews of adults within care home settings to ensure their needs are effectively met

* Perform Mental Capacity Assessments and make Best Interest Decisions to support individuals' care pathways

* Attend and actively participate in Continuing Healthcare Integrated Care Board assessments and reviews

* Complete Safeguarding Responses and Enquiries to ensure the safety and wellbeing of vulnerable adults

* Adopt strength-based and person-centred approaches to develop tailored care options for individuals

* Work collaboratively within a cross-functional team to deliver comprehensive care solutions
