Job Title: Adult Community Service - Team Leader, Autism Hub

Location: Guernsey (UK)

Salary: £44,918 Hourly

**What you will do:**

* Lead a dedicated team to create and implement effective support plans for individuals with learning disabilities and autism.

* Coordinate and manage diverse settings, ensuring 24-hour support is in place.

* Work with a multi-disciplinary team and associated agencies to deliver person-centred care.

* Perform assessments, develop care plans, and manage crisis interventions independently.

* Supervise and mentor junior staff, fostering a positive and supportive team culture.

* Maintain accurate and thorough records in line with regulatory and organisational standards.

* Participate in health and wellbeing activities with service users, fostering an inclusive environment.

* Oversee and administer medication according to policy guidelines.

* Attend to shifts according to the Duty Roster and be adaptable to changes in shifts when needed.
