Social Worker - Assessment & Intake Team in Southwark UK, earning £32 per hour. What you will do:

* Manage complex caseloads within the Older Persons and Physical Disability Service

* Conduct comprehensive assessments including Care Act, Carers, Mental Capacity, and CHC evaluations

* Present care packages to the panel for approval and arrange care home placements

* Stabilise complex cases and address initial safeguarding concerns

* Collaborate effectively with partner agencies, GP, Housing, and Occupational Therapy staff
