Manage a complex caseload, undertaking assessments, support planning, and reviews to promote independence and wellbeing</li><li>
Complete comprehensive assessments using a range of health and social care tools, including Continuing Healthcare assessments where appropriate</li><li>
Identify, coordinate, and facilitate access to Reablement and short-term support services</li><li>
Promote and apply a strength-based approach, supporting individuals to assess risks and make informed decisions</li><li>
Identify and utilise community, family, and natural support networks in line with the Adult Care Strategy</li><li>
Monitor and review care and support provision to ensure outcomes are achieved effectively</li><li>
Work collaboratively with hospital staff, local providers, and community services to improve outcomes</li><li>
Provide professional advice and guidance to service users, carers, and partner professionals</li><li>
Undertake safeguarding duties and specialist roles where trained and authorised</li><li>
Organise and prioritise workload effectively, maintaining confidentiality and professional standards at all times</li></ul>
